• I consent to my child receiving initial treatment from the school clinic by a qualified nurse, and if necessary at a local clinic or hospital. I agree to pay all related expenses.
• I understand I must provide a medical report to the school if requested.

• Copy of parent and student's Saudi/Iqama IDs
• Copy of student's and parent's passport
• Reference Letter from Sponsor (Guardian)
• Copy of birth certificate
• Copy of Immunization certificate
• Medical report from a licensed hospital (Grade 1/Year 2 or specific cases)
• Original previous school certificates/reports
• Financial clearance from previous school (KSA transfers)
• Noor Program transfer finalized (KSA transfers)
• Transcript equivalence from Ministry of Education (if from outside KSA)

Note: Failure to provide required documentation, or updated passport/Iqama copies, may result in withdrawal of the student.
